Mentor: Inservice/Stipend Request Form

FIRST YEAR REQUEST FOR PAYMENT
Full Year
Two to three (2-3) in-service credits for my work as a mentor during the school year. I will work at least 20-30 hours outside of the school day completing mentoring activities.

A stipend of $700.00 to be paid at the end of the school year. I will work at least 20-30 hours outside of the school day completing mentoring activities.

Half-year (1st year mentee hired between December 3 and February 15.
One to two (1-2) in-service credits for my work as a mentor during the school year. I will work at least 10-20 hours outside of the school day completing mentoring activities.

A stipend of $350.00 to be paid at the end of the school year. I will work at least 10-20 hours outside of the school day completing mentoring activities.

SECOND YEAR REQUEST FOR PAYMENT
Full-Year
One (1) in-service credit for my work as a mentor during the school year. I will work at least 10 hours outside of the school day completing mentoring activities.

A stipend of $300.00 to be paid at the end of the school year. I will work at least 10 hours outside the school day completing mentoring activities.

Half-Year
One half (.5) in-service credit for my work as a mentor during the school year. I will work at least 5 hours outside of the school day completing mentoring activities.

A stipend of $150.00 to be paid at the end of the school year. I will work at least 5 hours outside of the school day completing mentoring activities.
