Please read the following statements carefully and sign the bottom of this page.
As an AVID Tutor:
1. I understand that I am committed for the entire semester to my selected class(es). If, for some reason, I am not able to attend a session, it is my responsibility to inform the classroom teacher and/or lead tutor.
2. I agree to be a positive role model to the students in my class(es) in dress, attitude and academics. If I do not fully support the students in their endeavors, I understand that I may be asked to not return to the AVID class at any time.
3. I understand that I am required to attend a tutor training session at least once per semester.
4. I agree to maintain communication with my AVID classroom teacher(s) to provide the best assistance I can to the students.
5. I understand that it is my responsibility to submit an accurate time sheet in a timely fashion.
6. I understand that I will be expected to know and use the Cornell note taking system and a higher order of thinking during tutorial sessions.
7. I agree to abide by all district and site policies and procedures.
8. I understand that my job is to facilitate group learning activities—active participation is required.
9. I give permission to be photographed during tutorial sessions with students. Photographs may be used in AVID marketing materials in print and on the web.
