Instructions for Online Checklist Reporting
Work Sampling System Developmental Checklist

You will need an Infobase or Campus Username and Password for the on-line reporting process. If you dont already have one, send an email to sis.help@spps.org and indicate that you need a Campus Username and Password. If you have trouble logging on, contact the SIS Helpline at 603-4990.

Once you have your Campus Username and Password, go to http://mis.spps.org/. Look for Customized Web Sites where you will find STUDENT DATA. Click on the work sampling report listed here. Enter your User Name and Password as requested.

From the drop down lists, choose a calendar (school name). Many schools also have a calendar for learning center (LA), so if you dont see the list of students expected, double click your calendar choice.

Select a Term, Period, Grade, and Quarter, then click Go. Term and Period are used to create a list of teachers and their students. Grade and Quarter are used to choose which version of the work sampling form is used.

Choose your name/class from the drop down list and click Go.

You will see a list of students assigned to you in Campus. Click a student name.

The students name now appears on the right side of the screen and the appropriate work sampling form will fill in. Choose responses to the questions and click submit at the bottom of the page. Please note: Data is NOT SAVED until the submit button is clicked. After 20 minutes of inactivity, the pages will time out and unsaved data is lost. If you are interrupted from working on a student, you can save the data and return to finish later.

Near the top of the Checklist Form there are headings for the quarters. The first quarter, in caps, is the current quarter. To change to another quarter, you can click on the name of the quarter.

There will be two reports. Click REPORTS at the top of any students Work Sampling Checklist Report form. One prints all the reports with any data, one student per page. The second report prints a summary of all data collected on all of your students.
